Overview
The Joint Commission on Accreditation of Healthcare Organizations(JCAHO) published an article in their publication Sentinel Event Alert, Issue 25, 2/26/02 that studied a series of deaths and injuries related to long term ventilation. In an effort to prevent ventilator-related deaths and injuries, the task force identified 2 primary failure modes:
  • Failure to hear alarms
  • Failure to respond to alarms
